BYU-HAWAII ALUMNI NOMINATION FORM

Distinguished Alumni Award - awarded to alumni of BYUH who have: given outstanding service to their professions, community, nation or church, BYUH; and completed at least 50% of their educational program at CCH/BYUH or received a degree from CCH/BYUH.
Service to Family Award - awarded to alumni of BYUH for outstanding service, rendered in a home environment, including extended family. It may be given to an individual or a family.
Honorary Alumni Award - for those who have not attended BYUH but who have rendered outstanding and significant service to BYUH.
